Forms Required

These forms will help you ensure that your research is done safely and in line with scientific ethics.

Compliance will be required for any projects going on to the Canada Wide Science Fair (CWSF) and also the Intel International Science & Engineering Fair (IISEF.

1.BASEF Consent Form (Required)
This consent form must be signed by one of the student's parents and a school sponsor. Each student exhibitor must submit this form.

2.BASEF Safety Form
This form is to be used as a guideline for you during the development of your project. On Setup Day March 25, 2009, a BASEF Safety Inspector will use this form to complete a safety check on the project you have setup.

3. Scientific Review Forms (Required)

A. Junior Students
All Grade 7 & 8 students must print, complete and submit any applicable YSF Scientific review forms in order to enter BASEF and to be eligible for the Canada Wide Science Fair (CWSF).

Use the Checklist for Scientific Review Forms Grades 7&8 to help you to determine which forms are required for your project.

  1. Contribution from a Recognized Institution (Form YSF1)
  2. Designated Supervisor Form (Form YSF3)
  3. Use of Human Participants in Research Form (YSF Forms 4.1.1A, 4.1.1B, 4.1.1C and 4.1.1D; last pages of document)
  4. Use of Animals in Research Form (Form YSF4.1.2C; last page of the document)
  5. Continuation Projects Form (Form YSF7)

B. Intermediate and Senior Students
All Grade 9 to 12 Students must print, complete and submit any applicable IISEF Scientific Review Forms.

Use the Checklist for Scientific Review Forms Grades 9 to 12 to help you to determine which forms are required for your project.

  4. Registered Research Institutional/Industrial Setting Form (Form 1C)
  5. Qualified Scientist Form (Form 2)
  6. Risk Assessment (Phys/Chem) Form (Form 3)
  7. Human Subjects Approval and Informed Consent Form (Form 4)
  8. Vertebrate Animal Forms (Forms 5A-NON REGULATED or 5B-REGULATED)
  9. Potentially Hazardous Biological Agents Form (Form 6A)
  10. Human and Non-Human Vertebrate Animal Tissue Form (Form 6B)
  11. Continuation Projects Form (Form 7)

NOTE:
Projects submitted by Grade 9 to 12 Students are eligible for both the Canada Wide Science Fair (CWSF) and also the Intel International Science & Engineering Fair (IISEF). To meet both CWSF and IISEF requirements, grade 9 to 12 students are required to comply with all the relevant rules, for BOTH YSF and ISEF.

MAIL FORMS
Mail all your signed Scientific Review Forms and BASEF Consent Forms. The envelope must be postmarked no later than Saturday, February 28, 2009.
